Procedure
An upper eyelid lift can be safely performed in our Tysons Corner clinic under local anesthesia while you remain comfortably awake. The procedure itself typically takes about 45 minutes.
Upper eyelid concerns generally fall into three categories:
- Excess upper eyelid skin
- Excess upper eyelid fat
- A combination of both
On the day of surgery, Dr. Maia meticulously measures and marks the eyelids to ensure precision and symmetry. After antibiotics are administered and local anesthesia is injected, excess skin and fat are conservatively removed as needed. Great care is taken to preserve a natural amount of volume and avoid a hollowed appearance. The incision is carefully closed within the natural crease of the eyelid, where it heals as a thin line that fades beautifully over time.
Recovery
Sutures are removed approximately 7 days after surgery. The recovery process is generally well tolerated. We offer topical numbing for suture removal, although most patients find it unnecessary.
Initial swelling and redness gradually improve over several weeks, with final results becoming fully visible between 3 to 6 months as the eyelids settle and scars mature.

Procedure
Lower eyelid surgery is designed to improve under eye bags, puffiness, and loose skin that can develop with age. These concerns are often caused by shifting fat and changes in skin elasticity. The most common approaches include:
Scarless Lower Blepharoplasty
If under eye bags caused by fat pockets are the primary concern and skin quality is good, a scarless technique can be performed. A small incision is made inside the lower eyelid to remove or reposition fat. There is no visible external scar, and recovery is typically straightforward.
Skin Pinch or Skin Removal
If excess skin or mild crepiness is present, a conservative skin pinch may be performed to gently tighten the lower eyelid. In select cases, this can be done under local anesthesia.

Lower Blepharoplasty with Skin and Fat Correction
When both puffiness and loose skin need to be addressed, an incision is placed just beneath the eyelashes. Excess skin is carefully removed and fat is sculpted or repositioned. The incision typically heals as a fine line that becomes difficult to detect over time.
Fat Grafting and Nanofat Grafting
Many patients also experience hollowness or loss of support between the lower eyelid and cheek. In these cases, fat grafting restores volume, while nanofat improves skin quality and helps soften discoloration. These treatments are often performed in combination to create a smoother, more natural transition from eyelid to cheek.
If lower eyelid laxity is present, a canthopexy may be performed to provide additional support.

Recovery
Most patients take about one week off work when treating either the upper or lower eyelids alone. If upper and lower blepharoplasty are performed together, a two week recovery period is often recommended.
Swelling and bruising improve significantly within the first week, allowing most patients to comfortably resume social activities. Residual swelling continues to refine over several weeks as the final result gradually settles.
