Open Versus Closed Nose Job Surgery
It is important to understand the differences between open and closed nose job surgery. Open, or external surgery, involves creating a small incision on the piece of skin that sits between the nostrils to allow access inside the nose structure.
While the open approach leaves a very small scar, the advantage of this procedure is that the surgeon has much better access to the interior of the nose, improving the visibility of all the structures during surgery. With the closed or endonasal approach (scarless rhinoplasty), all incisions are made inside the nose, ensuring no visible scars. Dr. Maia offers both options (open and closed rhinoplasty) and will determine which option is best for you.
In either case, the surgery is performed under general anesthesia. For the sake of patient comfort, general anesthesia is usually preferable, although very simple procedures such as nasal tip refinements could be performed under local anesthesia. With general anesthesia, you will sleep through the surgery.
When the anesthesia has taken effect, Dr. Maia will make incisions to expose the nasal tissue being reshaped. If additional cartilage is needed in revision rhinoplasty (secondary rhinoplasty), it may be removed from the ear or rib during surgery. Rhinoplasty typically takes about two to three hours, but it could be longer, depending on the complexity of the procedure.
